To ensure eligibility for the program, please confirm the below statements are true and accurate:

  • The organisation is registered with the Australian Charities and Not-for-profits Commission (ACNC) and is endorsed as a Deductible Gift Recipient (DGR).
  • The organisation operates primarily in Australia and the contribution will benefit Australian based initiatives.
  • The contribution to the organisation will not benefit myself or my family.
  • The contribution to the organisation will not be used for a donation to another charitable organisation, or general overheads.
  • The contribution to the organisation will not be used for hazardous, dangerous or illegal activities, gambling, or cause harm to the community or any people or animals.
  • The organisation agrees to participate and cooperate as required in all promotional activities relating to the contribution, including but not limited to being interviewed and photographed by P&N Bank.
  • If the application is successfully shortlisted, the organisation will be accepting a financial contribution from P&N Bank, to be deposited into a nominated bank account for the purposes detailed on this form.
